Traditionally, dental care in the United States has been provided by independent private physicians. The American Dental Association (ADA) estimated that approximately 93% of all active dentists worked in private practices in 1998,15. This proportion has remained remarkably stable over the years. In fact, an essential characteristic of providing dental care in the United States is the diversity of modes of practice and their constant evolution within a philosophy of private practice. Adaptability in a rapidly changing world is one of the main strengths of private practice, an attribute that ensures that private practice endures.
Dentists in the military and the Department of Veterans Affairs are also often in hospital offices. Areas of interest in general dentistry must meet the Specified Criteria for Recognizing Areas of Interest in General Dentistry (PDF) to be formally recognized by the American Dental Association. In addition, some routine dental care is provided in hospitals for patients suffering from severe systemic illness and for whom the risk of being treated in the private dental office would be unacceptably high. As planned, primary health care in the dental office should begin with a focus on activities that directly impact oral health delivery. However, dental offices in department stores have not thrived and were few in the late 1990s.
